If you are looking to restore both volume and youthful positioning to your breasts, the combination of breast augmentation and mastopexy may be the right approach. It enhances breast size and shape while lifting and reshaping sagging breasts, for a more balanced silhouette.

Breast augmentation increases the size of the breasts using implants made of saline or silicone gel, placed either over or under the muscle depending on your anatomy and your goals. Mastopexy lifts and reshapes sagging breasts by removing excess skin and tightening the surrounding tissue, repositioning the areola for a more balanced look. Performed together, the two answer volume and position in a single surgery rather than two.

In select cases, Dr. Mafi may also reinforce the lifted tissue with GalaFLEX®, a bioresorbable scaffold that works like an internal bra. It provides temporary structural support to the breast tissue while your body's natural healing creates new collagen within the scaffold's structure. As the material gradually resorbs, what is left behind is your own remodeled, strengthened tissue rather than a permanent implant, and whether it belongs in your plan is part of the conversation at your consultation.

Planning starts with a thorough consultation. Dr. Mafi reviews your medical history, talks through the risks and what you can expect, and examines your breasts to assess skin quality, tissue elasticity and the degree of sagging. Photographs may be taken to plan the procedure. Together you settle on implant size, the lift technique that suits you, and where the incisions will sit: under the breast, around the areola, or a combination of the two.

The combination is typically performed under general anesthesia, so you are asleep throughout, and generally takes three to four hours depending on various patient factors. You go home in a surgical bra or bandages that support the breasts and limit swelling. Mild pain, discomfort and swelling are common in the first days, some bruising is normal, and heavy lifting and strenuous activity are to be avoided for one month.

Most patients resume regular activities within two to three weeks, full recovery takes about six to eight weeks, and the final shape reveals itself over several months as swelling subsides and the tissue settles.

The implant adds the fullness and the mastopexy raises and reshapes the breast, so both concerns are addressed in a single surgery instead of two.

Implants are saline or silicone gel, and they can sit either over or under the muscle. Which combination suits you comes down to your anatomy and the result you are after, decided with Dr. Mafi before surgery.

Incisions are typically made under the breast and around the areola, or in a combination approach in an anchor pattern. Scarring improves over time, and it is usually hidden under the breast and around the areola.

The change is long lasting, though aging and gravity keep acting on the breasts over the years, and a stable weight helps hold the result. When GalaFLEX® is part of the plan, the internal support it provides is designed to help the lifted shape hold while the scaffold is gradually absorbed and your own remodeled tissue takes over the support.
